Job summary

Hueman is looking for dedicated Nurse Practitioners to conduct in-home Health Risk Assessments for Medicare and Medicaid members in Jersey Shore, PA. This role provides flexibility in your schedule, allowing you to achieve your professional aspirations while connecting with patients on a personal level.

The ideal candidate will have a Master's Degree or equivalent experience and an active NP license. The position offers competitive compensation, travel reimbursement, and comprehensive training.

Qualifications

• Master's Degree OR sufficient experience and satisfactory completion of NP license.
• Active State NP license.
• Active National board certification. Current BLS certification required.
• 12 months of experience as a paid advanced clinical provider preferred.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation to ensure patient safety.
• Provide basic education on wellness.
