Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Corsicana, Texas. This role offers a lifestyle-friendly schedule and the chance to impact community health outside clinical settings.

Responsibilities include travel to patients' homes, comprehensive assessments, medical history review, and documentation. Qualifications require an NP license, board certification, and BLS, with 1 year of experience

Qualifications

• Master’s degree in nursing or equivalent with NP licensure.
• Active state NP license and national board certification.
• Current BLS certification.
• 1 year of paid clinical provider experience preferred but not required.

Responsibilities

• Travel to patients’ homes for HRAs.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document accurately.
• Conduct medical reconciliation for safety and compliance.
• Provide basic wellness and preventive education.
