New grads are welcome. The role includes 60 hours per month minimum, with travel across counties and malpractice insurance coverage.

Qualifications

• Active, unrestricted state NP license is required.
• Board certification: FNP, AGNP, or ANP is required.
• Reliable personal transportation is required.

Responsibilities

• Conduct pre-scheduled in-home health assessments for Medicare Advantage, Medicaid, and commercial plans.
• Complete pre-populated forms; no treatment plans or follow-up calls.
• Work across multiple counties within your region; travel as needed.
