Job summary

Advantmed is seeking a licensed Nurse Practitioner for 1099 PRN in-home wellness and risk-adjustment assessments across diverse patient populations. You will conduct in-home visits, document findings in EMR, and support value-based care with ongoing monthly availability.

The role offers flexible weekday scheduling, paid mileage, and dedicated scheduling support with travel within a 55‑mile radius.

Qualifications

• Active NP license in the applicable state.
• 3+ years patient care experience preferred.
• Comfortable with EMR systems and independent work.

Responsibilities

• Conduct in-home Annual Wellness Visits and chronic condition assessments.
• Perform focused histories, exams, vitals, medication reviews, and screenings.
• Educate members on preventive care and chronic disease management.
• Document visits in the EMR to support value-based care and risk adjustment.
