Job summary

Hueman is looking for dedicated Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) in Tionesta, PA. This role offers the freedom to prioritize personal preferences with a flexible schedule. NPs will enhance patient connections in a more intimate setting outside of conventional clinics.

Qualifications

• Master's Degree OR commensurate experience and NP license.
• Active State NP license and National board certification required.
• Current BLS certification necessary.
• 12 months experience as a paid advanced clinical provider preferred.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety.
• Provide basic patient education on wellness and preventative healthcare.
