Job summary

Collage Home and Community Rehabilitation Partners in Long Beach, CA, is seeking experienced Licensed Clinical Social Workers to provide patient-centered care in patients' homes and communities. PRN positions are available for clinicians who can support patients for the duration of their stay.

Our interdisciplinary teams create integrated treatment plans for neuro and complex ortho conditions, applying evidence-based practices in the patient's own home setting to achieve durable outcomes.

Qualifications

• Master's degree in Social Work required and LCSW licensure.
• Strong planning, organization, writing and time management.
• Ability to communicate with families, professionals and co-workers.
