Powerback Rehabilitation is hiring a Physical Therapist to deliver outpatient rehabilitation services in patients' homes across the United States. You will evaluate, plan, and provide PT programs to help older adults regain function and independence, with referrals to other disciplines as needed.

The role emphasizes patient-centered care, safety, and education for patients and caregivers, with a flexible schedule and travel between home visits.

Qualifications

• Must have a valid Physical Therapy license in the state of practice.
• Master's degree in Physical Therapy or equivalent.
• If degree earned outside the U.S., must be evaluated as U.S. equivalent.
• Driver's license or reliable transportation to patients' homes.

Responsabilités

• Provide outpatient physical therapy in home-based settings for older adults.
• Develop and deliver individualized therapy plans to restore and improve function.
• Refer to other disciplines as needed to address patient needs.
