Job summary

Brown University Health's Newport Hospital is seeking a Mobile Integrated Health Advanced Practice Provider to deliver home- and community-based care. The APP will work with physicians, nurses, care managers, social workers, paramedics, and community partners to improve outcomes and coordinate care for medically complex patients.

The responsibilities include comprehensive assessments, diagnosis and treatment within APP scope, care transitions, and telehealth-enabled monitoring.

Qualifications

• Current Rhode Island NP license or PA certification.
• NP: Master's or Doctoral degree in Nursing with Family Practice/AGNP/AGACNP certification.
• PA: Graduation from accredited PA program with NCCPA certification.

Responsibilities

• Perform comprehensive home and community health assessments.
• Diagnose, treat, and manage acute and chronic conditions within APP scope.
• Develop individualized treatment plans with patients and care teams.
• Coordinate transitions of care and referrals to services.
• Document in EMR and participate in quality initiatives.
