Paycom is seeking an experienced Medical Social Worker to provide in-home social work services in the Tampa area under the supervision of the Clinical Manager. You will assess psychosocial status, help develop and implement the care plan, coordinate visits, document changes, and support patients and families throughout discharge planning.

Prior healthcare experience and CSWE-approved MSW are preferred. CPR certification required; ACSW certification or in progress; strong communication and

• Master’s Degree required from a CSWE-approved program.
• ACSW certification required or in progress.
• Two years’ healthcare experience preferred (hospital/clinic/rehabilitation center).
• Criminal background check required.
• Current CPR certification required (Online certification is not accepted).

Responsabilités

• Provide medical social services in patients' homes per physician orders and under supervision of Clinical Manager.
• Assist admission by evaluating psychosocial status and home environment.
• Develop and implement interdisciplinary care plans as they pertain to medical social work.
• Observe, record, and report changes in emotional and social factors affecting care and treatment.
• Consult with attending physician regarding plan changes.
• Maintain and submit written clinical records: initial evaluation, care plan, and daily notes.
• Evaluate patient and family response to medical social work interventions.
• Coordinate scheduling of visits with Clinical Manager to facilitate staff visits.
• Participate in agency activities and staff development and in-service education.
• Supervise the social work assistant once monthly.
• Participate in discharge planning.
• Commit to ongoing professional development via education, workshops, and reading.
