Qualifications

• Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) preferred.
• Experience in home health or hospice strongly preferred.
• Ability to work independently in a field-based setting while supporting a Nurse Practitioner during patient visits.
• Must have reliable transportation and a valid driver's license.

Responsibilities

• Travel with and support the Nurse Practitioner during in-home patient visits.
• Assist with vital signs, patient history, medication reconciliation, and documentation.
• Prepare and maintain necessary clinical supplies for field visits.
• Facilitate communication between the provider, patient, and care team.
• Ensure accurate and timely documentation in the electronic medical record.
• Support coordination of care and follow-up needs identified during visits.
