Job summary

ExcelaCare Home Health in Hobbs, NM is seeking a Licensed Vocational Nurse (LVN/LPN) for full-time employment to administer skilled nursing care in patients' homes under RN supervision. You will coordinate with a multidisciplinary team, patients, families, and referring agencies to ensure continuity of care.

The role requires one year of nursing experience, an active LVN/LPN license, CPR, and a valid driver’s license with proof of auto liability.

Qualifications

• Graduate of an accredited School of Nursing for LVN or LPN.
• One year of nursing experience.

Responsibilities

• Administers skilled nursing care under RN supervision.
• Coordinate care with multi-disciplinary team, patient, and family.
• Document patient status and interventions timely and accurately.
• Participate in on-call rotation and visits as scheduled.
• Provide care per plan of care and agency policy.
• Assist RN/physician with end-of-life care duties.
