Job summary

ExcelaCare Home Health in Amarillo, TX seeks a Field LVN to provide skilled nursing care in patients' homes under RN supervision. This full-time per-visit role coordinates with the care team, patients, families, and referring agencies to deliver compassionate home health services.

The position requires graduation from an accredited LVN/LPN program, current TX LVN/LPN license, CPR, a valid driver’s license, and the ability to travel locally. Prior home health experience is a plus.

Qualifications

• Graduate of an accredited School of Nursing for LVN or LPN.
• One year of nursing experience.
• Current State LVN/LPN license; CPR; valid driver’s license and proof of auto liability.

Responsibilities

• Administers medications and treatments according to Agency policy and home care guidelines.
• Communicates effectively with the RN, patients, families, and physicians; documents patient status and interventions.
• Provides care under the supervision of the RN according to the plan of care established by the physician.
• Completes and submits documentation timely and accurately according to Agency policy.
