Job summary

Qualifications

• Graduate of an accredited School of Nursing for LVN/LPN.
• One year of nursing experience.
• Current State LVN/LPN license; CPR; valid driver’s license and auto liability proof.

Responsibilities

• Administers medications and treatments under RN supervision.
• Coordinates care with multidisciplinary team and patient/family.
• Documents patient status and nursing interventions in a timely manner.
• Participates in on-call rotation and home visits as scheduled.
• Assists RN/physician with end-of-life care when required.
• Upholds patient rights, confidentiality, and organizational policies.
