Aveanna Healthcare is seeking a Licensed Practical Nurse (LPN/LVN) to provide skilled home health care in the SA TX area. The role supports pediatric and adult patients with tracheostomy, ventilator and medication administration needs.

Training is available, and the position offers flexible scheduling including Friday–Sunday shifts. Ideal candidates hold an active LPN/LVN license, CPR certification, and are able to travel to patient locations.

Qualifikationen

• Must have and maintain an active, unencumbered LPN/LVN license in the practice state.
• Current CPR certification; Aveanna can assist in obtaining after hire if needed.
• TB skin test current within last 12 months.
• Six months hands-on nursing experience preferred but not required.
• Reliable transportation is required.

Aufgaben

• Deliver and coordinate quality patient care per physician orders.
• Observe, assess, and report changes in patient condition to supervisor/physician.
• Document all nursing actions and care coordination promptly.
• Participate in, implement, and update the nursing care plan.
• Take appropriate nursing actions based on assessment to achieve outcomes.
• Recognize changes requiring intervention and implement care to reduce risk.
• Comply with Aveanna policies, state/federal regulations and Nurse Practice Act.
• Provide care with infection control per OSHA standards.
• Educate patient and family on disease, self-care, and prevention.
• Maintain ongoing knowledge of nursing competencies through education.
