Aveanna Healthcare is seeking a Licensed Practical Nurse (LPN/LVN) in San Antonio, TX. The role supports pediatric and adult patients in a private duty home health setting with a focus on compassionate, high-quality care.

The ideal candidate holds an active LPN/LVN license, CPR certification, and at least six months of hands-on nursing experience. Training is available for acuity tasks, and schedule options include evenings with flexible shifts to fit availability.

Qualifications

• Graduate of an accredited school of nursing.
• Current, unrestricted state license as a Licensed Nurse in the state of practice.
• Current CPR certification.
• Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies.

Responsabilités

• Deliver and coordinate quality patient care in accordance with physician orders.
• Continuously observe patient condition and report changes to supervisor/physician.
• Document all nursing activities, assessments and care coordination promptly.
• Participate in and update the nursing care plan.
• Maintain infection control per OSHA standards.
• Educate patient and family on disease process, self-care and prevention.
