The role requires Clinic-to-community care, EMR documentation, and patient education. Travel up to 40% is required to serve diverse patient settings including homes and shelters.

Strong HIPAA compliance and bilingual skills are a plus. Ideal candidates hold a CA LVN license, CPR/BLS, and prior outpatient/home-health experience.

Qualificações

• California LVN license required.
• 1 year LVN experience in outpatient/home health preferred.
• Bilingual English/Spanish preferred.
• Willing to travel up to 40% across geography.
• Proficient with EMR/EHR and HIPAA regulations.

Responsabilidades

• Conduct in-home care visits and document observations in EMR.
• Perform vital signs, medical history, med reconciliation and report findings.
• Educate on chronic illness management and safe medication use.
• Provide palliative care to improve comfort and quality of life.
• Collaborate with multidisciplinary team to execute care plans.
• Engage in community outreach and patient enrollment assistance.
• Travel to patient homes and designated locations daily.
• Act as liaison between patients and providers.
