Job summary

Adobe Population Health is seeking a part-time Licensed Vocational Nurse (LVN) in Oroville, CA. The role involves in-home care, vital signs, education, and care coordination for patients with complex needs.

Travel across Butte County is required to deliver in-home support and collaborate with a multi-disciplinary team. The ideal candidate has CA LVN licensure, 1 year of LVN experience, CPR/BLS, and familiarity with EMR systems.

Qualifications

• Licensed LVN in California is required.
• 1 year of LVN experience preferred in outpatient/home health.
• Bilingual English/Spanish is a plus.
• HIPAA knowledge and patient confidentiality.
• Proficient with EHR systems and MS Office.

Responsibilities

• Perform in-home visits and chart observations in EMR.
• Take vital signs and collect medical history.
• Provide education on chronic disease management and safe meds.
• Deliver palliative care as needed to improve comfort.
• Coordinate with multidisciplinary teams to execute care plans.
• Assist with enrollment in community services as needed.
• Travel within Butte County and surrounding areas for patient visits.
