Job summary

St. Mary's Home Care, part of Trinity Health at Home, seeks a Home care LPN to deliver one-to-one nursing care in patients' homes across Yardley, Morristown, and Greater Bucks County. The LPN will perform assessments, care planning, and interventions under physician orders while following the Nurse Practice Act and agency policies.

Strong patient-centered focus, compassionate care, and flexible scheduling are essential to support our community-based care model and outcomes.

Qualifications

• Graduate of an accredited or state approved school of practical nursing.
• Licensure as an LPN in the State of Pennsylvania.
• Minimum one year of acute care experience; home care preferred.
• Phlebotomy and wound care experience required.
• Must have passed NAPNES or equivalent medication exam before administering medications.

Responsibilities

• Provide one-to-one care with patients in their homes.
• Follow physician orders and comply with Nurse Practice Act and policies.
• Assess, plan, intervene and supervise patient care and outcomes.
