Job summary

The VNA of Manchester and Southern New Hampshire is seeking a Licensed Practical Nurse to deliver home health care to patients of all ages. You will work under the supervision of Home Care RNs and the Clinical Manager to provide compassionate, skillful nursing care in patients' homes.

Responsibilities include assessments, care planning, documentation, and patient education. Valid NH LPN licensure and BLS are required; 2+ years of clinical experience, preferably in home care, is preferred.

Qualifications

• Graduate of an accredited school of Licensed Practical Nursing.
• Minimum of 2 years clinical experience, preferably in a home care setting.

Responsibilities

• Assesses patients’ physical and psychosocial status and home environment.
• Reviews medical data prior to delivering care and documents patient status.
• Assists in developing patient care plans under supervision of the RN.
• Communicates information to patients, families, and other care teams.
• Incorporates problem-solving and quality improvement into patient care.
• Delivers nursing care with dignity and a therapeutic environment.
