Job summary

Elliot Health System in Manchester, NH, seeks a licensed practical nurse to deliver in-home health care to patients of all ages. You will assess physical and psychosocial status, contribute to care planning, and collaborate with Home Care Nurses to provide patient-focused care in the home setting.

Responsibilities include documenting patient status, coordinating with physicians and family, and ensuring understanding of insurance coverage and costs.

Qualifications

• Graduate of an accredited school of Licensed Practical Nursing required.
• Minimum of 2 years previous clinical experience required, preferably in a home care setting.
• LPN NH licensure required. Current BLS certification or must obtain within 90 days of date of hire required.
• Schedule may include eight to twelve hours per day with weekend, evening, and night coverage.
• Valid driver's license to travel throughout southern New Hampshire; proof of automobile insurance; motor vehicle background check will be conducted.

Responsibilities

• Assesses patients’ physical and psychosocial status and home environment.
• Documents and communicates patient status and assesses needs to Home Care Nurses.
• Participates in development of a patient care plan to facilitate health care and wellbeing.
• Provides nursing care while promoting patient dignity and a calm, therapeutic environment.
