Job summary

The Visiting Nurse's Association of Manchester and Southern New Hampshire is hiring a Licensed Practical Nurse for full-time day shifts in home health care. You will assess patients at home, coordinate with Home Care RNs, and help develop care plans while ensuring comfort and safety for clients of all ages.

The role requires NH LPN licensure, current BLS within 90 days, and at least two years of clinical experience, preferably in home care.

Qualifications

• Graduate of an accredited school of Licensed Practical Nursing.
• NH LPN licensure required; BLS within 90 days of hire.
• Minimum of 2 years in clinical care, preferably home health.

Responsibilities

• Assesses patients' physical and psychosocial status at home.
• Documents patient data and communicates status to Home Care RN.
• Participates in developing a patient care plan with RN guidance.
• Explains insurance coverage and cost of care to patients and families.
• Provides nursing care in a calm, supportive home environment.
