Job summary

Iowa Home Care in Polk County, West Des Moines, IA is seeking a Licensed Practical Nurse (LPN) to provide in-home care across client visits. You will implement physician-directed care plans, perform nursing procedures, and collaborate with an interdisciplinary team to maintain client health and independence.

Qualifications

• Must hold an LPN license issued in Iowa.
• 1–2 years of experience in acute hospital or home health care preferred.
• Knowledge of nursing standards as set by the Iowa Board of Nursing.
• Strong communication and ability to maintain confidentiality.

Responsibilities

• Deliver care according to physician’s plan and nursing standards.
• Conduct home visits, assess client conditions, record vitals, and note changes.
• Administer nursing procedures and manage medications per physician orders.
• Educate families on home safety, infection control, and fall prevention.
• Document care accurately and timely; coordinate with RN Case Manager.
