Qualifikationen

• LPN licensed in Iowa.
• 1-2 years of experience in acute hospital or home health care preferred.
• Knowledge of nursing standards as set by the Iowa Board of Nursing.
• Strong communication skills and commitment to confidentiality.
• Valid driver’s license and reliable transportation.

Aufgaben

• Provide skilled nursing services in patients' homes and during scheduled shifts.
• Follow physician orders, individualized care plans, and nursing standards.
• Assess patients and monitor vital signs, symptoms, and overall condition.
• Recognize and report changes in patient status to the RN Case Manager and care team.
• Administer medications and treatments within scope of practice.
• Perform nursing procedures appropriate to patient needs and licensure.
• Assist patients with maintaining a safe, comfortable home environment.
• Teach patients and family about medications, treatments, infection prevention, fall prevention, and home-care basics.
• Maintain complete, accurate, and timely clinical documentation.
• Communicate with RNs, physicians, caregivers, and the interdisciplinary team.
• Participate in coordinating services and identifying solutions to support care goals.
