Qualifications

• Active South Carolina LPN license required.
• Prior home health, long-term care, or clinical nursing experience preferred.
• Strong knowledge of medication administration, wound care, and patient assessment.
• Excellent communication and interpersonal skills with patients and families.
• Reliable transportation and a valid driver’s license for home visits.
• Ability to work independently and manage time effectively in a field-based role.
• Compassionate, patient-focused demeanor committed to quality care.

Responsibilities

• Monitor and document patient health status, vital signs, and progress toward care goals.
• Communicate effectively with patients, families, and the interdisciplinary care team.
• Educate patients and caregivers on disease management, medications, and healthy practices.
• Maintain accurate and timely clinical documentation in compliance with home health regulations.
• Identify changes in patient condition and report to supervising RN or physician promptly.
