Job summary

UnitedHealth Group is seeking a Home Health LPN/LVN to deliver skilled nursing care in patients' homes under RN supervision. You will verify plans of care, collaborate with physicians, and educate patients and families on self-care.

Responsibilities include vitals, medication reconciliation, and documentation. Eligible candidates have 1+ year of LPN/LVN experience, an active license, CPR, and a valid driver’s license with reliable transportation for home visits. Benefits are included.

Qualifications

• 1+ year of LPN or LVN experience.
• Current LPN license in the state(s) assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify when condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
