Job summary

Elk Valley, part of LHC Group within the Optum family, is seeking an experienced Home and Community Based Services Licensed Practical Nurse to provide direct nursing care in patients' homes, under supervision.

You will document visits in the EMR, support care coordination, and ensure high-quality patient outcomes in the service area. This role offers a collaborative environment and opportunities for professional growth.

Qualifications

• Current and unrestricted LPN licensure in the state of practice.
• Current CPR certification or ability to obtain within 90 days of hire.
• Valid driver's license and vehicle insurance, access to a dependable vehicle.
• Ability to function in home settings regardless of client demographics.
• Preferred: flexible hours, ability to manage multiple tasks, independence, strong communication, writing and organizational skills.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state laws and plan of care.
• Documents patient visits thoroughly in the EMR using the agency's electronic visit verification program.
• Participates in care coordination, reports changes in condition, attends case conferences, and communicates with patient, family and team.
