You will work under the supervision of the RN or supervisor to ensure quality patient outcomes.

Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current CPR certification.
• Current driver's license and access to a dependable vehicle or public transportation.
• Ability to function in any home situation regardless of client characteristics.

Responsibilities

• Provides high quality clinical services per plan of care and infection control procedures.
• Documents patient visits thoroughly in the electronic medical record.
• Coordinates care, reports changes in condition, and communicates with patient, family, physician, and care team.
