Job summary

Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current driver?s license and vehicle access, or reliable transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws and plan of care.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination, communicating with patient, family, physician and team.
• Able to function in any home situation regardless of client demographics.
