Job summary

Eminence Home Care is seeking a Licensed Nurse Assistant (LNA) to support home-based Personal Care Services in New Hampshire. You will collect information, observe functional abilities, and document findings for review by a supervising nurse.

The role requires NH LNA licensure, completion of approved training, and ability to travel within the service area. Experience in home care is preferred, and there are opportunities for ongoing education and professional growth.

Qualifications

• Current, unrestricted New Hampshire LNA license in good standing.
• Completion of a New Hampshire Board of Nursing-approved nursing assistant education program and demonstrated competency within the LNA scope of practice.
• Current CPR and first aid certification, if required by agency policy or contract.
• At least one year of experience in home care, long-term care, community health, or services for older adults or individuals with disabilities preferred.

Responsibilities

• Schedule and complete assigned client visits within required timeframes.
• Verify client identity and protect client rights during visits.
• Observe and document functional performance related to bathing, dressing, grooming, etc.
• Record the home environment for accessibility and safety.
• Report urgent concerns to the supervising nurse as required.
