Job summary

Eminence Home Care is seeking a licensed nurse assistant (LNA) to join our team in New Hampshire. The LNA will support personal care services, observe functional abilities, and document findings for review by a supervising nurse, under NH law and agency policy.

The role involves travel within the service area, adherence to confidentiality, and participation in ongoing training and orientation. Strong observation, communication, and documentation skills are essential for quality home care.

Qualifications

• Current, unrestricted New Hampshire LNA license in good standing.
• Complete NH Board of Nursing-approved nursing assistant education program.
• CPR and first aid certification.
• At least one year of experience in home care, long‑term care, community health, or services for older adults or individuals with disabilities preferred.
• Successful completion of required background checks, exclusion screening, health screening, and onboarding requirements.
• Valid driver’s license, reliable transportation when travel is essential.
• Ability to use electronic documentation systems and mobile technology securely and accurately.

Responsibilities

• Schedule and complete assigned client visits within required timeframes.
• Verify client identity, explain purpose and limits of the visit, obtain acknowledgments, protect client rights.
• Observe and document home environment for accessibility, safety, sanitation concerns, and barriers to care.
• Record client routines, preferences, strengths, limitations, and family supports.
• Report urgent concerns to supervising nurse and contribute to documentation and care planning.
