Job summary

Hueman is actively hiring Nurse Practitioners and Physician Assistants to conduct in-home Health Risk Assessments for Medicare and Medicaid members in and around Roanoke, VA. This role offers a flexible schedule and the chance to impact patient care outside traditional clinic settings.

Qualifications

• Master's degree in nursing or commensurate experience with NP/PA licensure.
• Active State NP/PA license and National board certification.
• Current BLS certification.
• 12 months of paid advanced clinical provider experience preferred.
• Ability to travel to patients' homes and perform non-invasive assessments.

Responsibilities

• Travel to and from patients' homes for HRAs.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document thoroughly and timely.
• Provide basic patient education on wellness and prevention.
