Hueman is actively hiring Nurse Practitioners and Physician Assistants to perform in-home Health Risk Assessments for Medicare and Medicaid members in and around Heatherville, VA. This role offers a flexible schedule and travel to patients’ homes, with comprehensive home assessment training to support your success.

Responsibilities include non-invasive physical assessments, medical history review, documentation, and patient safety through medical reconciliation, plus patient education on

Qualifications

• Master’s Degree or commensurate experience and NP/PA license required.
• Active State NP/PA license required.
• Active National board certification required.
• Current BLS certification required.
• 12 months of experience as a paid advanced clinical provider preferred but not required.

Responsabilités

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.
