Grant Memorial Hospice, part of the LHC Group, offers in-home hospice social work services across the medical spectrum. The Social Worker provides medical social services under physician direction to support patients and families through emotional and social challenges related to health status, working within an interdisciplinary team.

This full-time role requires a Master's Degree in Social Work and an active Social Work license, with travel within a 50-mile service radius and standard business

Qualificações

• Master's Degree from a school of Social Work accredited by the Council on Social Work
• Licensed as a Social Worker in the state of residence

Responsabilidades

• Assists the patient, significant others, physician and health care team staff to understand significant personal, emotional, environmental and social factors related to the patient's health status on an as needed basis
• Contributes as a health care team member to the development of a comprehensive, integrated Plan of Care for patients on a daily basis
• Instructs health care team members on community resources available to assist patients on a as needed basis
