Caretenders Hospice is seeking an RN Case Manager to provide in-home hospice care. You will deliver the Plan of Care for hospice patients, collaborate with clinicians, and ensure quality of care. On-call evenings and weekends, travel within a 50-mile radius, and standard hours Monday–Friday, 8:00 AM–4:30 PM. Ohio RN licensure is required; Home-Based Hospice experience preferred. Hourly pay ranges from $43.10 to $64.65.

Qualificações

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ years of clinical experience.
• CPR certification or ability to obtain within 90 days of hire.

Responsabilidades

• Make the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identify the patient's/family's needs and reassess every 15 days.
• Initiate and coordinates the plan of care.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with the patient/family, attending physician and IDG daily.
• Instruct and supervise the patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes regarding the patient's condition.
• Work in home settings regardless of age, race, creed, color, sex, disability, or financial condition.
