Job summary

St. Croix Hospice in Missouri seeks a Registered Nurse Case Manager to deliver 1:1 home-based care, partnering with an interdisciplinary team to develop individualized plans and guide end-of-life conversations. This full-time role is 8:00am–5:00pm with on-call duties.

Requires an accredited RN degree and 1+ year of acute care experience; hospice/home health is preferred. Reliable transportation and a valid driver’s license required.

Qualifications

• RN with accredited nursing degree (AA/BSN/MSN) required.
• 1+ years of acute care experience required; hospice/home health preferred.
• Ability to assess patients, develop care plans, and educate caregivers.

Responsibilities

• Conduct comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patients and families.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
