Job summary

St. Croix Hospice is seeking a dedicated Registered Nurse Case Manager to join our team in Harrisonville, MO. In this full-time role, you will provide compassionate, 1:1 support to patients and families in their homes, develop individualized care plans, and guide end-of-life conversations.

Qualifications

• Must have 1+ years of recent acute care experience, preferably hospice/home health.
• Requires reliable transportation and valid driver's license.
• Education in nursing from an accredited program is necessary.

Responsibilities

• Provide 1:1 holistic support to patients and families in their homes.
• Develop individualized care plans with input from patients and families.
• Administer medications and educate patients on care techniques.
