Job summary

Eden Health in Pocatello, Idaho seeks a Home Health and Hospice Registered Nurse (RN) on a PRN basis to provide in-home services to patients under a plan of care. The RN will manage caseloads, assess symptoms, coordinate with the interdisciplinary team, educate families, and ensure quality, compassionate care from admission through discharge.

Requirements include an accredited nursing program graduate, current RN license, hospice experience, valid driver’s license, CPR, and ability to pass

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in the state of practice.
• One year of Hospice experience within the last 24 months.
• Valid driver’s license and reliable transportation.
• Proof of automobile insurance (MVR checked).
• Proof of current CPR certification.
• Able to pass criminal background check and national sex offender clearance.

Responsibilities

• Provides in-home nursing services per plan of care.
• Manages caseloads with 12-18 patients depending on complexity and setting.
• Demonstrates competency in pain management, symptom control, wound care and aseptic technique.
• Coordinates with interdisciplinary team to maintain continuity of care and attend group meetings.
• Educates patients and families on rights, responsibilities, and care plans.
