Job summary

Your Health Organization is seeking a Continuous Care Nurse to serve patients in the Sumter area with in-home, short-term, bedside clinical care. This full-time role supports the care team to stabilize symptoms, ensure comfort, and maintain safety while adhering to all laws and policies.

The Nurse Case Manager will handle a full patient load, traveling daily to homes and facilities. A valid driver’s license and reliable transportation are required to meet the region’s needs.

Qualifications

• Must be a licensed nurse (RN preferred).
• Minimum three (3) years clinical experience preferred.
• Experience in community settings recommended.
• Proven ability to communicate and collaborate with care teams, patients, and caregivers.
• Strong written and verbal skills and basic computer knowledge.

Responsibilities

• Provide in-home intensive, short-term bedside clinical care to manage uncontrolled symptoms.
• Collaborate with the interdisciplinary hospice team to stabilize symptoms and ensure comfort, dignity, and safety.
• Develop and implement Care Management Plans of Care for eligible patients.
• Coordinate care and communicate with patients, families, and providers.
• Document Care Management activities accurately in the patient record.
