Job summary

Your Health is seeking a Registered Nurse to join our field-based Continuous Care team. The role focuses on in-home, short-term bedside care to stabilize symptoms and ensure comfort, dignity, and safety for patients in Florence and Sumter, SC.

As a Nurse Case Manager, you will collaborate with an interdisciplinary hospice team, manage a full patient load in the region, and travel daily between offices, facilities, and homes. A valid RN license and strong communication are essential.

Qualifications

• Must hold a valid RN license in good standing.
• Minimum 3 years clinical experience preferred.
• Experience in community/home-based care is preferred.
• Strong written and verbal communication skills required.
• Must be able to travel daily and manage multiple visits.

Responsibilities

• Facilitate receiving medical records from patients’ providers and specialists.
• Review medical records and complete consents.
• Enroll patients in Care Management when eligible and initiate a Plan of Care.
• Coordinate and communicate care with interdisciplinary teams and families.
• Document Care Management activities accurately and keep plans up to date.
