Job summary

Quality Home Health, Hospice & Palliative Care is seeking a compassionate Hospice Chaplain/Spiritual Care Counselor to serve patients and families across the Wasatch Back, including the Park City area. This paid-per-visit role provides mileage reimbursement for work-related travel and supports the interdisciplinary hospice team with spiritual care, bereavement, and end-of-life comfort.

Qualifications

• Graduate of an accredited seminary or theology school or chaplaincy education/certification.
• Two years of chaplain or spiritual counselor experience preferred.
• Hospice, healthcare, chaplaincy, bereavement, or end-of-life experience preferred.
• Current driver’s license and reliable transportation.
• Ability to provide spiritual care that respects diverse beliefs.

Responsibilities

• Provide individualized spiritual and emotional support to hospice patients and families.
• Develop and implement spiritual plans of care and communicate needs to the hospice team.
• Participate in the interdisciplinary group (IDG) and collaborate with care staff.
• Support families before and after patient death as appropriate.
• Coordinate with community clergy and spiritual resources when requested.
