Pathways Healthcare in Kingston, PA seeks a Chaplain to provide direct spiritual care to hospice patients and families in Luzerne and Lackawanna Counties on a per diem basis.

The role includes assessment, care planning, and collaboration with the interdisciplinary team to meet identified spiritual needs, plus liaison work with community clergy. Per visit rate, mileage reimbursement, 401(k) match, and flexible schedule are provided.

Formación

• Graduate of accredited seminary or school of theology with BD, MDiv or equivalent.
• Experience working with death and dying in hospice preferred.
• Ability to work as part of an interdisciplinary group.
• Comfort in an interfaith setting and respect for diverse beliefs.

Responsabilidades

• Assesses spiritual needs of patients and families and ensures timely services.
• Participates in comprehensive assessment to develop individualized care plans.
• Provides direct spiritual care to patients and families.
• Acts as liaison to community chaplains and spiritual counselors.
• Maintains records of spiritual care services for quality improvement.
• Documents direct services and communications with chaplains.
• Attends patient care conferences as part of the interdisciplinary team.
• Provides consultation, education, and support to the team.
