Job summary

Pathways Healthcare, based in Kingston, PA, seeks a Hospice Chaplain to provide direct spiritual care to patients and families within Luzerne and Lackawanna counties. You will collaborate with an interdisciplinary team, assess spiritual needs, and coordinate appropriate services in a compassionate, culturally sensitive manner.

You will network with community clergy, plan memorial services as needed, and participate in ongoing quality improvement activities to support exemplary end-of-life care.

Qualifications

• Graduate of accredited seminary or school of theology with BD, MDiv or equivalent.
• Ability to work as a member of an interdisciplinary team.
• Ability to network with community clergy and congregations.
• Knowledge of and commitment to hospice philosophy of care.
• Demonstrated compassion and cultural sensitivity during end-of-life care.

Responsibilities

• Assure spiritual assessment of patients and families and coordinate timely services.
• Develop individualized plans of care addressing identified spiritual needs.
• Provide direct spiritual care to patients and families.
• Serve as liaison to community chaplains and spiritual counselors.
• Maintain records of spiritual care services for quality assurance and program development.
• Document direct services and ongoing communication with chaplains and counselors.
• Attend patient care conferences as part of the interdisciplinary team.

Job Description:

• Role: Hospice Chaplain
• Main Office location: Kingston, PA
• Service Area: Luzerne and Lackawanna Counties.

Qualifications:

• Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree). Experience working with death and dying, individuals/family/caregiver.
• Ability to work as member of interdisciplinary group.
• Ability to accept different lifestyles, cultures, beliefs, and values.
• Ability to network with community clergy and congregations.
• Knowledge of and commitment to hospice philosophy of care.
• If making home visits, must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
• Hospice/experience preferred.
• Provides funeral or memorial services for patients as requested.
• Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
• Assists in supervision of spiritual care volunteers when assigned to patients/ families/caregivers.
• Actively participates in quality assessment performance improvement teams and activities.

Responsibilities:

• Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
• Facilitates the development of the individualized plan of care by participating in comprehensive assessment to meet identified spiritual needs.
• Provides direct spiritual care to patients and families/caregivers.
• Serves as liaison and support to community chaplains and spiritual counselors.
• Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
• Documents direct services and ongoing communication with community chaplains and spiritual counselors.
• Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group.
