Job summary

Crossbridge Hospice is seeking a compassionate Hospice Aide to provide in-home care in Flint, MI. You will work within a multi-disciplinary team to support patients in their residence, helping with personal care and comfort as outlined in the Plan of Care.

Ideal candidates are CNAs with valid driver's license and ability to work in field settings; high school diploma or GED preferred. The role offers flexible PRN hours and opportunities for growth within a patient-centered organization.

Qualifications

• Must be a certified nurse’s aide (CNA).
• State Tested Nursing Assistant (STNA) for Ohio.

Responsibilities

• Provide compassionate personal care to patients in their residence and support the Plan of Care.
• Assist with at least 2 ADLs including personal hygiene.
• Assist patients with medication intake and prescribed exercises.
• Follow the Plan of Care and update during scheduled visits.
• Be responsible to ensure the use of the 4Ms and provide Age-Friendly Care.
