Job summary

Crossbridge Hospice is seeking a compassionate Hospice Aide to provide in-home personal care and supportive services for patients in Flint, MI. The role focuses on comfort, dignity, and quality of life, following an individualized Plan of Care established by the care team.

Qualified candidates are CNAs (STNA in Ohio) with a valid driver’s license and reliable vehicle. Prior experience in home-based or hospice care is preferred, and teamwork within a multidisciplinary setting is essential.

Qualifications

• Must be a certified nurse’s aide (CNA). State Tested Nursing Assistant (STNA) for Ohio
• Must maintain a valid driver's license, good driving record and reliable automobile
• High school graduates or GED preferred
• Demonstrate the ability to be able to work within a multi-disciplinary team setting
• Demonstrate the ability to work in field situations

Responsibilities

• Provide compassionate personal care and supportive services to patients in their place of residence
• Assist individuals nearing end of life to maintain dignity, comfort, and quality of life
• Deliver care aligned with the patient’s Plan of Care and care team directives
• Follow the Plan of Care and update during scheduled visits
• Uphold 4Ms framework: What Matters, Medications, Mentation, Mobility

How You Will Contribute

The Hospice Aide provides compassionate personal care and supportive services to patients in their place of residence. This role assists individuals who are nearing end of life, helping maintain dignity, comfort, and quality of life. The Hospice Aide is an essential member of the care team and delivers care in alignment with the patient’s individualized Plan of Care.

How You Will Make an Impact

• Provide assistance with at least 2 of the following ADL's including personal hygiene
• Assist patients with medication intake and prescribed exercises
• Incidental household services, which are essential to patient health care
• Will follow the Plan of Care (Instruction Sheet) as instructed and make appropriate updates during scheduled visits
• Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care

The Experience and Skills You Bring
