Well Care Health seeks a compassionate Home Health Registered Nurse to deliver patient care in home settings, educating families and coordinating with a multidisciplinary team. You will assess conditions, plan and evaluate care, and manage a caseload across multiple life stages from neonates to geriatrics in accordance with agency standards.

Applicants should be licensed in the state(s) of practice, CPR certified, and have at least 1 year of RN experience with 2+ years in clinical settings.

Formación

• RN license in North Carolina or SC with active status.
• CPR certification required.
• 1 year RN experience and 2+ years total clinical experience.
• Home health experience preferred.

Responsabilidades

• Assess and monitor patients in their home environment.
• Develop and implement individualized care plans.
• Educate patients and families on health management and self-care.
• Collaborate with a multidisciplinary team to optimize outcomes.
• Manage a caseload and coordinate care per agency policies.
