Job summary

Humana Inc. CenterWell Home Health in Texas is seeking a Home Health Registered Nurse to provide one-on-one in-home skilled nursing, administer medications, and help patients regain independence in the comfort of their homes, in the Midland/Odessa area.

You'll develop care plans with physicians, educate families on disease management, monitor progress, and coordinate with the Clinical Manager and a team of clinicians to ensure high-quality, compassionate care.

Qualifications

• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Med surg, ICU, ER, or acute care experience.
• Valid driver's license and reliable transportation.
• Preferred: one year nursing experience.
• Preferred: Home Health experience.

Responsibilities

• Provide skilled nursing care to patients, working onsite in their homes, including administering medications, performing wound care, and conducting patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes.
• Educate patients and their families on disease management, medication, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care and condition progress to patient's physician and Clinical Manager.
• Oversee the work of Home Health Aides and monitor their appropriate completion of documentation.
• Use your skills to make an impact.

Skills

Med surg, ICU, ER, or acute experience

Current CPR certification

Experience collaborating with a team

Valid driver's license

Education
