Humana Inc. is seeking a CenterWell Home Health Registered Nurse to provide one-on-one skilled nursing care in patients' homes. You will develop care plans with physicians, monitor progress, and educate patients and families while coordinating with the broader care team.

The role emphasizes collaboration with clinicians, documentation accuracy, and adherence to safety programs, including TB screening and driving requirements. This is a full-time, on-site position with a 40-hour workweek.

Formación

• Must hold a current, unrestricted RN license.
• CPR certification is required.
• Experience collaborating with healthcare professionals is needed.
• Valid driver's license, auto insurance and reliable transportation required.

Responsabilidades

• Provide skilled nursing care at patients' homes, including medications, wounds, and assessments.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patient conditions and report changes promptly.
• Educate patients and families on disease management and treatments.
• Maintain accurate care records and coordinate with the care team.
• Report progress to the patient’s physician and Clinical Manager.
• Oversee Home Health Aides and review documentation completion.
• Utilize skills to make a meaningful impact on patient recovery.

Conocimientos

Med surg ICU ER experience

RN licensure

CPR certification
