Job summary

Humana Inc. is seeking a Home Health Registered Nurse to join CenterWell Home Health. In this role you provide one-on-one skilled nursing care in patients' homes, administer medications, perform wound care, and develop individualized care plans with physicians and clinicians.

You will monitor patient conditions, educate families on disease management, and coordinate with the care team. The position requires driving to patient homes, valid license, and CPR certification.

Qualifications

• Med surg, ICU, ER, or acute experience.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Provide skilled nursing care to patients in their homes.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes.
• Educate patients and their families on disease management, medication, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care and condition progress to physician and Clinical Manager.
• Oversee the work of Home Health Aides and monitor their documentation.
