Clearwater is seeking a qualified Registered Nurse to provide skilled nursing care in patients' homes in the Clearwater, FL area. You will perform comprehensive nursing assessments, coordinate services with the care team, and ensure accurate documentation and adherence to Medicare/Medicaid guidelines.

The role requires Florida RN licensure, 1+ year experience in home health or hospital settings, and strong organizational and communication skills.

Qualifikationen

• Must be a graduate from an accredited School of Nursing.
• Must be licensed in the state of Florida, as a Registered Nurse.
• One or more years of experience in community/home health agency or hospital setting preferred.
• Knowledge of Medicare and Medicaid guidelines.
• Working knowledge of home healthcare and required documentation.
• Strong organization and time-management skills; effective communication with nursing management.

Aufgaben

• Provide nursing care per patient plan of care.
• Coordinate admission, transfer, re-certification, resumption of care, and discharge OASIS for home health patients.
• Consult with attending physician regarding alterations to Patient Care Plans and update supervisor.
• Coordinate patient services and submit daily tally of visits.
• Submit clinical notes and progress notes within 48 hours.
• Assist in discharge planning and provide patient teaching to patients and families.
• Supervise Home Health Aide and evaluate their work every 14 days.
