Job summary

Qualifications

• Must be a graduate from an accredited School of Nursing.

Responsibilities

• Coordinate total patient care and perform comprehensive health and psychosocial evaluation.
• Evaluate nursing service effectiveness for patient and family.
• Complete admission, transfer, re-certification and discharge OASIS.
• Prepare and present patient records to committees as indicated.
• Consult with physicians on care plan changes and coordinate with the health team.
• Coordinate patient services and participate in staff development.
• Maintain knowledge of current drug therapy and ensure regulatory compliance.
